Permission to Share Information

There are many programs and services offered to students by the Millard Schools. Some of these programs provide assistance for students who qualify for free or reduced price meals. These programs, however, cannot access your children's eligibility status without your consent.ÌýIf you are interested in receiving assistance for your children for any of the programs listed below, you can complete the attached permissionÌýto share form and return to your child's school building or mail to:

The Summer Food Service Program provides free meals to all children ages 1 through 18 years at participating sites statewide. No paperwork or pre-registration is required to participate.ÌýUse any of the following tools to find an SFSP site near you:
